Declaration for Invalid Pension State of Ohio, County of Carroll
Personally appeared, Albert Tipton, aged 56 years, a resident of Perry
Township, County of Carroll, State of Ohio, who being duly sworn,
declares: that he is the identical Albert Tipton who
enrolled on the 15th day of May 1864, a Corporal, Co. I 157th O.V.I.,
in the War of the Rebellion and served at least ninety days,
and was honorably discharged at Camp Chase, O on the 2nd day of
Septmber 1864. That he is unable to earn a support by manual
labor by reason of Rheumatism. That his Post Office address is: Lamartine, Carroll County, Ohio.
Signed: Albert Tipton Witnesses: Jarrett Morgan, Joseph Kirby; both residing at Lamartine,
Ohio, who declare that they have been acquainted with the claimant for
forty years and forty five years, respectively.

Personally appeared, Albert Tipton, aged 61 years, a resident of the
township of Perry, county of Carroll, State of Ohio, who declares:
that he is partially unable to earn a support
by manual labor by reason of Rheumatism and Heart Disease, Dizziness of
the Head. That he has heretofore applied for a
pension under application number 1,071533 and rejected pension
application of March 6, 1894. That is Post Office Address is:
Lamartine, Carroll County, Ohio. He hereby appoints with full
power of substitution and recovation, Wm. H. Host of Bowerston, Ohio
his true and lawful attorney.

General Affadavit of Physician
State of Ohio, County of Carroll; In the matter of Albert Tipton, Co. I
157th Ohio Infantry, certificate 950571; in the township of Lamartine,
county of Carroll, State of Ohio
We have this day examined said applicant and
found him as follows; walk about 60 feet and pulse was 130, very
peculiar vibrations of strain of heart which we cannot account
for. Has had rheumatism in shoulders and back for years, has
vertigo and has to have head elevated when sleeping. Think him liable
to death at any time from heart failure.
Is a farmer and do not think him able to do manual labor; now is able to perform 1/4 or 1/8 of an ordinary man.

That he is 71 years of age, a resident of
Carrollton, County of Carroll, State of Ohio. That his personal
description at enlistment on the 2nd day of September 1864 at Camp Chase, Ohio was:
Height: 6 feet, complexion dark; color of eyes: grey;
that he was born July 17, 1835 at Lamartine, Carroll County, Ohio;
that since leaving service, his places of residence have been:
Have always lived in Carroll County in the townships of Perry and Center. That he is a pensioner and is drawing $12. per month.

Regarding pensioner Albert Tipton.
Sir: I have the honor to report that the above
named pensioner has been dropped from the rolls because of information
received from the postmaster in Carrollton, Ohio that the
pensioner died 6 Feb 08 (Note: wonder if the date of death is correct,
since it is date stamped Feb 1909)
